Overview As a member of the interdisciplinary team, the Social Worker provides services to respond to the emotional needs of residents and their families. This employee also assists with resident admissions and the referral process. Responsibilities As part of the admission process, meet with the resident and family to review policies on advanced directives, resident rights, and discrimination; assist in completing admissions paperwork as needed for newly admitted residents and families. Conduct and document a social services evaluation, including identification of resident problems/needs; provide/arrange for social work services as indicated by resident/family needs, transfers, and changes of level assignment. Apply professional standards of social work practice, establish criteria to ensure services meet established standards of quality, and interact with residents, families, community, and interdisciplinary staff. Demonstrate assessment skills to evaluate residents' behavior, psychosocial needs, risk factors, and responses to interventions; interpret and explain resident behavior to staff to foster understanding, respect for resident rights, and minimize the use of restraints where appropriate. Complete the RAI process in a timely manner and maintain/update care plans; coordinate and facilitate family meetings to keep residents and families informed of condition changes and interventions. Implement social service interventions that achieve treatment goals and link social supports, physical care, and environment to enhance quality of life. Track PASARR screenings, OBRA screenings, and other related mental health screenings to assist the resident in maintaining good financial status and nursing home eligibility. Express self adequately in written and/or oral communication and communicate effectively in an interdisciplinary care setting with residents, families, and staff. Carry out all duties in accord with the facility mission and philosophy; respect the rights, dignity, and individuality of each resident; maintain confidentiality of resident and family information; demonstrate honesty and integrity in the care and use of resident and facility property. Ensure the quality and appropriateness of resident care meets or exceeds company and industry standards; ensure services comply with state, federal, legal, regulatory, accreditation, and reimbursement guidelines. Maintain effective relationships and open communication with residents, families, and staff. Qualifications Minimum of bachelor's degree in Social Work; Master's Degree preferred. One year supervised social work experience in a long-term or sub-acute care rehabilitation facility required. Currently licensed as Social Worker in the State of Michigan. Knowledge of regulatory standards and compliance requirements. Working knowledge of personal computer and software applications (word processing, graphics, databases, spreadsheets, etc.). Strong organizational, analytical, oral communication, and written communication skills. Working knowledge and ability to apply professional standards of social work practice. Current knowledge of state and federal laws and regulations applicable to the practice of social work in long-term care. Language Skills Ability to read, analyze, and interpret professional journals, technical procedures, or governmental regulations; ability to communicate orally and through written reports and other documents; and ability to present information and respond to questions from groups of executives, managers, clients, customers, and the public. Reasoning Ability Ability to define and solve problems, collect data, establish facts, and draw valid conclusions; ability to interpret complex instructions and deal with several variables. Physical Demands The physical demands described here are representative of those that must be met by an employee to perform the essential functions. The employee is occasionally required to stand and walk; the employee is frequently required to sit, use hands and fingers to handle or feel, reach with hands and arms, and talk or hear; occasional physical effort with light to medium objects weighing 10 to 15 pounds; vision requirements include close, distance, and peripheral vision.
